Terms of Service

1. Who we are

HeartFit For Life is a program of the Cardiac Therapy Foundation, 4000 Middlefield Rd. Suite G-8, Palo Alto, CA 94303. You can reach us at support@heartfitforlife.org or 650-494-1301.

2. This app is not medical advice

The app helps you record information and stay in touch with your care team. It does not diagnose, treat, or replace the judgment of your physician or nurse. Alerts and summaries in the app are aids, not clinical determinations. Always follow the instructions your care team gives you directly.

3. Who may use it

Access is by invitation to enrolled HeartFit members and program staff. You must be 18 or older. Your account is yours alone — please do not share your sign-in with anyone.

4. What you record

You are responsible for the accuracy of what you enter. Readings you record yourself, or that arrive automatically from a connected device, may be incomplete or wrong. Your care team may rely on them, so tell them if something looks incorrect.

5. Connected devices

You may choose to link a fitness or health device, such as Fitbit. Doing so lets us import activity and heart-rate data into your record. Those devices are made by other companies under their own terms, and we do not control their accuracy or availability. You can disconnect a device at any time from within the app.

6. Your health information

How we use and protect your health information is described in our HIPAA Notice of Privacy Practices, which forms part of these terms.

7. Acceptable use

Please do not attempt to access another person's record, disrupt the service, or use it for anything other than your own care or your role in the program.

8. Availability

We aim to keep the app running, but we cannot guarantee that it is always available or free of errors. We may change or discontinue features.

9. Ending access

You may stop using the app at any time. We may suspend access if you leave the program or misuse the service. Your health record is retained in accordance with our Notice of Privacy Practices.
